Sonya wrestles with treating a guy from her past and must rely on Amy to help her. Richard (Scott Wolf) deals with a family situation. Jake and TJ debate the moral implications of a diagnosis in the all-new “Secrets and Lies” episode of Doc.

Lying in Amy’s bed before work, Jake helps Amy study for the medical boards.
Arriving for work, Amy and Jake walk into the hospital elevator together, when Michael walks in. It’s awkward for Amy and Jake, but Michael doesn’t suspect anything. They make small talk about how Katie is in DC. Jake asks about it, although he is obviously aware that Katie is in DC, and Michael says she’s at a debate tournament with her school. Amy says that Katie is a junior in high school.
Sonya walks into her patient’s room and instantly freezes up. We find out that she was sexually assaulted by her patient, Ravi, who tells Amy that he and Sonya dated five years ago. Sonya calls Richard to tell him about the situation and ask to be excused from the case, but he doesn’t respond to her calls or texts.
Sonya struggles emotionally throughout the episode. Ravi baits her with vile comments about how she wanted to sleep with him and felt guilty after, so she made it seem like rape. Traumatized by the rape flashbacks she keeps getting and Ravi’s disgusting comments, Sonya snaps and puts air into his IV. Ravi codes. Amy comes in to help manage the code, and Sonya tells her what she did. Amy tells Sonya to go home for the day, but doesn’t tell anyone about what happened.
At the end of the episode, Sonya thanks Amy for being there for her and wishes her luck on the medical boards. Amy tells her to get some professional help. Sonya takes some time off work to deal with her trauma.
Throughout the episode, we see Richard and his wife Victoria struggling with their son Lucas’s mental health. At the end of the episode, Lucas packs up to go away and get help at an inpatient facility.
As Lucas packs up, Richard tells his wife Victoria that he wasn’t there for Sonya when she really needed him because he was so distracted by everything going on with Lucas. He tells her that it wasn’t the first time he missed something important because of Lucas’s mental health issues…he also made a mistake a few months ago that killed a patient (Bill Dixon). Richard says he’d be finished professionally if Amy ever remembered what happened. He clearly feels remorse about the situation, and Victoria hugs him.
The episode ends with Amy sitting down to take the medical boards. Katie mentioned to Michael in Season 1, Episode 6 that Amy was taking the boards “next week,” grounding the timeline of this episode.
